2024-2025 Rogers ISD Health Information:

If your child needs to take a daily prescription drug or over the counter medication during school hours, you must submit the medication in its original container and submit a Request for Administration of Medication Form. Both parent and physician must sign the form (available online or upon request from the school nurse). Please do not send medication to school with the student- a parent or legal guardian must bring the medication to the front office and provide the order and medication in it's original container.

   Important Reminders

• Please fill out the RISD Health Information card for each child with current phone numbers, student health conditions and medications.
• Please submit an updated shot record to the school nurse anytime your child receives a vaccination. It is state statute that if your child does not have all necessary immunizations, your child will not be allowed to attend school until proof of immunization is provided. 

If your child is sick:

• Please do not send your child to school if he/she is running a fever of 100 or greater. Your child may return to school when he/she is free of fever for 24 hours without the help of fever reducing medications.
• Please do not send your child to school if he/she is vomiting or has diarrhea. Your child may return to school when he/she is no longer vomiting and/or has diarrhea for 24 hours. 
• If your child has pink eye(conjunctivitis), student may return to school once they have been on antibiotics for 24 hours. 
• If your child has head lice, please notify the school nurse and begin treatment. Student may return to school once treatment has begun and student has shown improvement. Please see the lice management guidelines tab for more information.
